- Subgingival Irrigation treatment (Peridex)
- Doctor exams ( up to 2 per year)
- 1 Emergency exam and x-rays per year
- X-rays as needed (annual and/or panoramic)

- Digital photos
- Up to 15% off dental services including SRP (deep cleaning), Arestin (gum antibiotic), cosmetic, restorative, CBCT scan, surgical and implant procedures

Terms and Conditions:
- Our membership plan is not a dental insurance plan but a membership payment plan provided by a single dental office and is not transferable to other dental offices.
- Membership premium is due at the time of the first appointment.
- Payment for any cosmetic, restorative and/or surgical treatment is due at the time of service.
- Membership dues must be current in order to receive plan benefits.
- Membership plans may not be combined with any other discounts.
- It is solely the patient’s responsibility to schedule and keep their appointments.
- No refunds will be provided for dues paid under any circumstances, including failure to schedule and maintain appointments.
- Monthly dues and fees for dental services may change at any time.
- Membership will automatically renew on the anniversary date unless cancelled 30 days previous in writting.
- Membership services that are not used are lost, they are not carried over to the following year.
